Appointment Scheduling

 

1. If your insurance requires a prior authorization for a specialist,  please obtain this as soon as possible and ask for it to be faxed to our office prior to the appointment.

2. Bring to the appointment:     

 

  • Prior authorization (if needed) 
  • Insurance Card 
  • Picture ID 
  • Co-Pay 
  • If your child has been having problems in school, please bring any school reports you have when you come for the evaluation. This includes: Report cards, IEP's, Teachers reports and/or Standardized test results: Cognitive (IQ), Achievement, Adaptive, etc.
  • Teacher's Vanderbilt behavior form completed.

3. New patients please arrive 30 minutes prior to the appointment.

Important information for your appointment:

1. Appointment and cancellation: Our services are by appointment only. Your appointment represents valuable time for both you and your provider and we require that you notify us at least 24 hours in advance of a cancellation or change. Our answering service will not take messages regarding cancellations or prescriptions. Please call during office hours. A fee of $100.00 will be billed to you if you fail to keep your appointment or do not give the necessary 24 hour notice. Insurance companies do not pay for missed appointments, so you will be responsible to pay this before your child can be seen again.

2. Arriving to the appointment: For new patients, we ask that you arrive 30 minutes prior to your scheduled appointment. If you are a returning patient, please arrive at least 15 minutes prior to your appointments. Each visit requires a brief questionnaire to fill out prior to seeing your care provider. Co-payments, deductibles and outstanding balances are due at the time of every visit. If you are 15 minutes or more late for your appointment, we may have to reschedule for another date and time. It is always recommended that you call in these situations so we can make adjustments if necessary. We do our best to stay on time with the schedule, but occasionally a provider may go over the allotted appointment time. Please know that every child is important to us and the same special attention will be given to your child.Please make arrangements for child care for siblings for at least the first 3 initial evaluation appointments. Other children in the office, particularly younger, frequently make the evaluation more challenging. Thank you for your understanding.

3. Providers: After the initial evaluation, your child may continue his/her follow up appointment with a Nurse Practitioner.

4. Prescription Refill: If your child has not been seen within 6 months, we will not be able to provide medication refills until your child has come into the office for a visit. It's important for your child to be consistent with the prescribed medications. Interruptions to the medication schedule can cause adverse reactions for your child. We strongly encourage you to keep your scheduled appointments or reschedule within enough time so there won't be any interruptions we require 48 hours notice to process all prescription refills and it may take 72 hours before it is mailed out. Please be certain to contact our office in advance to allow up to 72 hours turn around time. Thursday and Friday prescription request will be processed on the following Monday and Tuesday.

5. Provider Phone calls: When calling our office to speak with a care provider, please be aware that they are seeing other patients throughout the day, and often not able to respond immediately to telephone calls.  If your call is urgent, please make this is clear in your message. If you are in an emergency situation please call 911 or go directly to the nearest emergency room. Most crisis in this field of service warrant taking your child to the emergency room, for further tests and care.

6. Payment: As a courtesy, we will submit claims to your insurance. Please note that some diagnoses or tests may not be included in your policy, therefore payment may be denied by your insurance provider. Co-payments, deductibles and unpaid visits are your responsibility and payable at the time of service.  It is your responsibility to know what your insurance does not cover.  If you have concerns about specific tests or treatments, please feel free to contact our office for the case specific codes that your insurance may require.

7. Medical records:  If you need copies of your child's medical records, there is a charge of $1.00 per page due upon picking those records up. A medical record release must be signed to release any records.

8. Additional Payments: There is a charge of $25.00 for any letters dictated or written by our Providers. This will be billed to your child's account. Please submit payment prior to your next appointment.
